SHIPPER'S INSTRUCTIONS FOR DESPATCH

COMPLETION OF THIS DOCUMENT IS NECESSARY FOR CARGO LINE INTERNATIONAL PTY LTD TO COMPLETE AN AIR WAY BILL ON YOUR BEHALF PLEASE BE ACCURATE AND USE BLOCK LETTERS.

The Shipper or his Authorized Agent hereby authorizes CARGO LINE INTERNATIONAL PTY LTD in his name and on his behalf, to prepare any export documents to sign and accept any documents relating to said shipment and forward their shipment in accordance the conditions of carriage and the tariffs of the carriers employed. The shipper guarantees payment of all collect charges in the event of the consignee refusing payment. Hereunder, the sole responsibility of Cargo Line is as indirect carrier subject to its conditions of carriage and the sole responsibility of Cargo Line is as agent for the direct carrier after issuance of direct carrier's waybill.

Failure to complete this question may result in delays in moving your goods. The Shipper certifies that the particulars on the face hereof are correct and agrees to be bound by the company's Standard Terms and Conditions which appear on the reverse hereof and that insofar as any part of the consignment contains Dangerous Goods such part is properly described by name and is in proper condition for carriage by sea according to the International Air Transport Association's Dangerous Goods Regulations.
